**Job Title: Case Manager - Substance Use Disorder (SUD)**

**Position Overview: **
We are seeking a compassionate and detail-oriented Case Manager specializing in Substance Use Disorder (SUD) to join our dedicated team. The ideal candidate will be responsible for providing comprehensive support to individuals struggling with substance use issues, ensuring they receive the necessary resources and guidance to achieve their recovery goals.

**Key Responsibilities:**

- Assess client needs through interviews and evaluations to develop tailored case management plans that address both substance use and co-occurring mental health disorders.
- Provide ongoing support and guidance to clients throughout their recovery journey, including crisis intervention, resource navigation, and motivation for behavioral change.
- Collaborate with multidisciplinary teams, including counselors, therapists, medical providers, and community organizations, to coordinate comprehensive care for clients.
- Maintain accurate and up-to-date case files, progress notes, and documentation in accordance with legal and ethical standards.
- Monitor and evaluate client progress, modifying case management plans as necessary to ensure optimal outcomes.
- Facilitate support groups, educational workshops, and community resource sessions to empower clients and promote recovery-oriented practices.
- Advocate for clients' needs and rights, ensuring access to appropriate services, support systems, and community resources.
- Stay current with best practices, treatment modalities, and legal regulations related to substance use disorders.
- Participate in case reviews, team meetings, and ongoing professional development opportunities.

**Qualifications:**

- Bachelor’s degree in social work, psychology, counseling, or a related field (Master’s degree preferred).
- Relevant certification in substance use disorder counseling, case management, or related areas is a plus (e.g., CADC, CAC, or similar).
- Minimum [2] years of experience working with individuals with substance use disorders and/or in case management.
- Strong knowledge of SUD treatment modalities, community resources, and recovery support practices.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work empathetically and effectively with diverse populations.
- Proficient in electronic health record (EHR) systems and case management software.
- Valid driver's license and reliable transportation may be required.
